Here are my requests:

1) Pay at least $200 on every 13th and 27th of the month

2) Sign and notarize a document stating the following:

- Due to acts of deception you committed in 2012 and 2013, you owe the PokerFraudAlert community approximately $12,200 plus interest

- You have agreed and committed to make payments of at least $200 toward that amount on the 13th and 27th of every month without fail, until it is paid off

- You will provide these payments via Paypal, cash, or money order to Todd Witteles, who will hold the money to be properly distributed to the community at a later point in time

- If you can afford more than $200 twice per month, you will send that additional money to Todd Witteles in order to pay it off faster. This means you will not use your income to gamble, take vacations, purchase gifts, or purchase non-essential items. Any disposable income will be sent to Todd Witteles in order to get everyone paid off in a more rapid fashion
