Insurance companies look at Medicare and if they pay, they will pay. At this point individual insurance companies have outlived their usefulness.
Medicare decides what a doctor can do for an individual and if they don’t, they don’t get paid.
There is an entirely new level of scam going on with these Medicare advantage plans. People that get others to sign on get paid on a rolling basis.

Despite the differences in backpack color and what some believe to be different facial features, the two pictures released are of the same guy.
They know a lot about him now, but not his name, and they still don't have a full, unhooded facial pic of him.
He is not from NYC. He came into town on November 24, stayed at a hostel, checked out for 3 days, then came back there. He had some roommates (strangers) at the hostel, who have not said much about him yet, or at least that hasn't been released. He took a taxi to McDonald's at one point. He checked into the hostel with a fake New Jersey ID. He apparently had the mask and hood on the entire time -- something which didn't arouse suspicion because of the cool weather in NYC.
The best shot of him -- the smiling photo -- was due to either an inability to say not to a girl, or fear of bringing on suspicion. The girl at the front desk at the hostel was having a conversation with him, and said, "Come on, let's see what you look like... pull down the mask!" At that point, he did pull down the mask and gave that big smile. That's the clearest photo authorities have of him. It's unclear if he just couldn't say not to the (presumably young and pretty) female desk clerk, or if he was afraid that refusing to show his face would cause her to report him to authorities. I'm guessing it's more likely the latter.
The hostel would suggest he doesn't have much money. He would have had fewer witnesses staying in a hotel without any roommates, even a cheap one.
He looks young in the pictures.
"Deny, defend, depose" actually has a second meaning -- it seems to be a modification of the title of a book criticizing the insurance industry, called "Delay, Deny, Defend". Someone on Reddit posted a link to the book in PDF format.
Again, unless this is an intentional false lead, this hit was clearly some sort of revenge about a denied claim. Given that the shooter looks young, it's likely about a relative, such as his parent.

Absolutely ridiculous that this is the lead story across the country, even on local stations. This guy might be getting more coverage than the assassination attempts on a Presidential candidate received, especially the 2nd at the Golf Course.
People get targeted and shot daily, especially in the ‘hood. Little effort goes into investigation of these murders let alone pics being broadcast across the nation.
