Disaster.

Stanislav Zegal had KQ and Daniel Neilson had AK, and they got it in on a QhKh5d board.

Neilson had 53m in his stack, Zegal had 38m.

Neilson lost the hand and should have been left with 15m, but a counting error had the dealer claiming Zegal had 48m, so Neilson was charged an extra 10m for losing the hand. This left Neilson with just 5m chips, and he went out 3rd.

The ICM calculations showed that this stiffed Neilson out of $116k in equity, and indeed he finished in 3rd for $900k -- the worst possible finish at that point.
