You're looking at this the wrong way.
COVID spreads much more easily in a population which hasn't been infected much yet. The big cities got hit first, yes, because it was easier to spread there due to population density.
After that, eventually the non-hard-hit populations start getting it, which is what's been happening since November, due to the cold weather sending people indoors more often.
This isn't a Republican versus Democrat thing. This is a COVID fatigue thing, where people are hanging out again because they're tired of being isolated for all this time. Laughably, a lot of the Democrats hanging out together don't wear masks, either, because "we are responsible and safe and wear masks everywhere else, so none of us have COVID, unlike those stupid Republican rubes".
This is why you see so many Democratic politicians being caught in situations they've banned or deemed dangerous, and never wearing a mask themselves.
You can't just write off other parts of 2020, and point to November-December as "proof" that Republicans are the ones getting it. A lot of the blue states were already hit hard, and the ones that weren't (Minnesota, California, etc) are suffering now.
BTW, "total cases per 1M pop" is meaningless, because there was far less testing earlier in the year, so hard-hit early-year places like NYC were FAR under-tested.
8 of the top 12 deaths-per-1M states went for Biden, with the top 3 all being places where Biden won easily.

The states with the smallest populations are the ones with the highest deaths per million. Look at blue Rhode Island and Mass.
Not listed on that chart are the blue states of NY, NJ, Connecticut, Michigan, PA, which have some of the highest deaths per million.
When you look at all states and not a cherry picked list, 8 out of the top 12 states in deaths per million went Biden.
Trying to pin this on red states is just not accurate.
Yes, covid is real, yes, you don't want it, but lockdowns don't work, kids should be in school, and blue states are destroying themselves in ways that almost feel intentional.
