In 2012, he posted a thread entitled "Threat to Todd Witteles", where he laid out a bunch of demands that I tell the "truth" about what happened between me and toots in Texas in 2008, or he would post a ton of personal/family info here.
The "truth" wasn't all that exciting -- and I later told it on the forum anyway. Obviously when he posted this threat, I banned him. However, I chose to quietly do so, rather than make a spectacle of it at the time. I never understood why he did this, because he was never close with toots or any of her friends. However, he was permabanned for doing this, and I was never going to let him back.
Oddly enough, he never posted anywhere that he was banned, nor did he try to contact me again. I figured he knew exactly why he was banned, was embarrassed, and just decided to forget about PFA. Then he vanished from all forums, and that seemed to be that.
About a year ago, he contacted me, asking me to reset his password because he forgot it. He had no idea I had ever banned him!
I told him what he had done back in 2012, and he seemed genuinely shocked. He said he had no memory of such a thing, but that he believed my story. He apologized, said he's a different person now, and asked me to forgive him for it. I told him that it had been 8 years, and nothing further happened beyond this "threat", so I would forgive him and let him back on.
However, he never responded to that message, and I haven't heard from him since.
On a side note, as I mentioned last year, I happened to be placed at the same starting table as toots in last year's "Big 50" event at the WSOP. We spent a full 20-minute break talking, and there's no bad blood between us. We didn't even mention the shit from back in 2008, just talked about what we've been doing lately. She showed me pictures of her kids, I showed her pictures of my son, and she busted shortly after the break. I ended up cashing in 666th (out of 28,000), and I was busted by the hand 666. I'm sure that was some kind of sign.
