I wonder if he was HIV+. The studies I have seen indicate T cell loss and T cell exhaustion is highly correlated with disease prognosis; and HIV+ definitely exhausts your CD8 T cells prematurely, in addition to wiping out many of your CD4 T cells.
I remember when COVID first hit there was some stories about young healthy looking (almost assuredly gay) dudes getting real sick and dying, and I was speculating they were HIV+.
